Traditionally, dolls have been considered therapeutic tools primarily for children, often within the context of play therapy. However, recent research indicates that symbolic objects such as dolls can also serve as effective therapeutic mediators for adults. This paper explores the psychological, neuroscientific, and clinical foundations of doll-based interventions with adults. Findings suggest that dolls can facilitate emotional regulation, trauma processing, and self-objectification, ultimately enhancing resilience and therapeutic outcomes.

Play therapy has long established the value of dolls and symbolic play in child psychotherapy. Yet, adults also engage with symbolic objects—such as collectible dolls, action figures, or art toys—in ways that resonate with identity, attachment, and memory. Despite cultural biases that associate dolls with childhood, adults’ interaction with dolls may activate mechanisms of projection, externalization, and narrative reconstruction, providing therapeutic benefits.

Psychological Mechanisms

1. Projection and Externalization

Adults, like children, may project internal conflicts onto dolls, using them as external containers of emotion. This process reduces psychological defense and enables safe exploration of vulnerable feelings (Winnicott, 1971).

2. Transitional Object and Attachment Theory

From the perspective of attachment theory, dolls may function as transitional objects, fostering comfort and security in times of distress. In adulthood, this function extends to coping with trauma, grief, or loneliness.

3. Narrative Identity Reconstruction

Engaging with dolls in therapeutic contexts allows adults to construct alternative life narratives, providing symbolic distance from painful experiences and enhancing self-coherence.

Neuroscientific Basis

Neuroscience suggests that embodied interaction with symbolic objects activates brain regions involved in empathy, memory, and self-processing (mPFC, ACC, insula). Touch and manipulation of dolls may regulate stress responses by lowering cortisol levels, similar to mechanisms observed in tactile therapies. Furthermore, mirror neuron systems may be engaged when adults role-play or project scenarios through dolls, enhancing emotional attunement.

Clinical Applications

1. Trauma Therapy

Dolls can provide a safe medium for survivors of trauma to reenact and reframe distressing experiences without direct verbal confrontation.

2. Grief Counseling

Dolls representing lost loved ones can function as symbolic surrogates, aiding in the mourning process.

3. Elderly and Dementia Care

Doll therapy has shown promise in reducing agitation, anxiety, and loneliness in older adults with dementia, improving emotional well-being (James et al., 2006).
